Maybe the rear brake was dragging. The resulting heat ruined the bearing and the dragging brake ruined the rotor??

If the OP means that by going out he means that the balls came out of the bearing completely, I would assume the axle would be loosely floating around in the hub causing the rotor to possibly get warped???

When a ball bearing starts going south. Excessive play would begin as the balls come out of cages or lose their spacing along inner and outer races.. Some movement should have been felt by rider on smaller bike. Bearing going into total destruct mode likely from over loading or lack of lube.
